Differentiated learning as it appears on Mindsteps Inc.

The Mindsteps plan differentiates between four types of learners, and calls them HCHP, HCLP, LCHP, and LCLP.  Those things obviously stand for something, but they won't tell me what.  The Reference Guide describes what the four types of learners need in terms of content, process, product, and environment.  The sample Planning sheet is a template that breaks a lesson plan down into six parts, with places to figure out how to modify each part for each of the four learner profiles.  This Calculus Differentiated Unit Plan is an example of one lesson plan filled out with all the differentiated instruction.  (Differentiation in calculus!  Woohoo!)
